AND DOCUMENT REQUEST

PLEASE BRING ONLY THOSE THAT APPLY TO YOUR CASE FROM THE FOLLOWING LIST:
- All Forms W-2 and Forms-1099 provided to you by your employer (s)
- Last year’s tax returns Form-1040 & California Form-540 (for new clients)
- All other Forms-1099 for other income i.e., interest from banks, credit union, escrow, dividend income, misc. income, property sale, abandonment, short-sale etc.
- Escrow closing papers for your purchase, sale, refinancing of home/property
- Purchase contract for car used in business or self-employment
- All Forms-1099 for IRA, Keogh, Pensions, Annuities withdrawn/distributed
- Statements for social security income or retirement benefits received
- Total business income and summary listing of expenses during the year
- Car mileage records, business travel expenses, company reimbursements
- Car expenses (if used for your employer) and company reimbursements
- Medical expenses, i.e., insurance, prescription medicines, doctor/dentist
- Taxes paid: real estate taxes, State taxes, DMV fees, sales taxes, property taxes
- Charitable contributions: church, goodwill, used car, used clothes, etc.
- Interests paid and insurance in your mortgage, home equity, refinancing (Form-1098)
- Lender’s Statement on Property Foreclosure, Cancellation of Debt (1099A,C)
- Other deductibles: union dues, uniforms, tools, tax preparation fees, legal fees, misc.
- Child and Dependent Care payment: name, address, social security number
- IRAs/Keogh/Pensions: amount contributed, converted, and when
- Rental income and list of rental property-related expenses during the year
- Name of dependents, birthdays, relationship, social security number, Tax ID
- Business bank account deposits and bank charges during the year.
- Hybrid vehicles purchase tax credit certificate from dealer and energy-efficient home improvements
- Gambling winnings from the casinos, etc. and deductible gambling losses
- Theft and/or casualty loss records from fire, flood, earthquake hurricane, tornado, etc.
- Uncollected loans to private persons or personal debts that did not get repaid
- Real estate property sale and/or foreclosure records, if any
- Stockbroker stock sales summary and stock purchase records summary, if any
- Car mileage record logs for car/vehicle used in business or employment or job-related trips
- Business assets purchase and depreciation records, Forms-K1 from other business activities such as partnership, LLC, S-Corp, Trusts, etc.
